By whom (?who? - never did get that one sorted), FFS? I can't recall the last time I encountered that form of address to any woman of my personal acquaintance. I am literally amazed to learn it's still widespread.
Actually, I've just realised that while I'd never dream of using "mrs hisname hisname" when addressing just Her, I have automatically addressed christmas cards to "Mr & Mrs hisinitial surname". I was taught that was how you did it, back in about '60. Luckily they're all to people who think that's how you do it. From now on I'll just leave out the initial. "Mr X and Mrs Y Z" is just clumsy. I /think/ for all the couples I know where each has kept their own surname, the envelope just gets both personal names. In the order I met them in originally, usually.
