My only thought is that Linnea hasn't been defined in the early parts of the plan - she is only mentioned by name. In the post-delivery you mention 'Older daughter'.
It may well be worth making the link between the two more explicit - especially if this is to be given to strangers for their records in advance.

Once my birth plan had been read and signed by the relevant parties, I found it relatively easy to stand my ground (over the isotonic drinks thing, I mean), because in my head, the question had already been settled.
(Is there an 'of' missing in the clause about "siting IV drips", or is "siting" adjectival here?)
I only got as far as the General section, then realised I couldn't possibly read the rest of it - but I feel the tone you set in the part I read is appropriate. You seem to be making it clear what you require and why, in a polite but firm manner.
