Letter to the hospital

OK, this is something I've been trying to do for well over a year now, and I can't. I want to write to the hospital and get a full copy of my notes, as per Data Protection act or whatever it is. I want the notes from ob/gyn and the colo-rectal guy and the dermatologist.

And I can't compose this simple little letter.

Does anyone want to write it for me? Linnea was born on 30 April 2004.

As per (DPA - relevant act), I would like a copy of all the notes associated with the maternity and birth of (Linnea's name) born on (date).

I have made an auumption you were cared for at the Royal Berkshire. If so here is the form to complete and send back in. There is nor need to write a letter.
