Your obstetrician sounds rather like the one I had in my first pregnancy! I think your idea of a plan and a back-up plan is good: a normal birth OR a C-section, with no chance of a half-way house. I had an emergency C-section with my daughter (now 16) and would have preferred to have been awake - it's very odd to go to sleep pregnant and wake up with a baby.
